Is this bugged? Cant seem to get it to procc

What world tier does it drop from?

Got my first one in Tier 3 world, in a box from event, in a dungeon

for everyone thinking this is bugged: it procs on a lucky hit, and only on skills that innately have bleeding from the skill, like battle lunging strike, furious whirlwind, rend, flay, etc.

I don't think using something like HotA with the 2h sword expertise equipped would trigger this since the skill doesn't innately have bleed, nor do skills that can out use a 2h sword like death blow. I can only get it to trigger using lunging strike, rend, ww, etc.
