I was also able to become an Oathbreaker as an Order of the Ancients Paladin by killing Pandirna (The Tiefling in the storage shack at the druids camp that is paralyzed). Not completely sure if that is supposed to happen or not though.

As a devotion paladin, I was able to convert to Oathbreaker by torturing the guy in the goblin camp. I had the option even after setting him free.

It says it's a channel oath charge, so it stands to reason that it would replace either the "heal allies nearby" ability oath of ancient paladins get or the "make melee attackers take 1D4 radiant damage" ability oath of devotion get, but not divine smite. As for who it gives advantage to, it specifically says "attack rolls against it have advantage" and not "attack rolls you make against it have advantage", so I would read that as anybody making an attack against a creature affected by it gets advantage on all attacks until the effect ends. Pretty powerful ability compared to "3 healing to anybody within 3 meters of you", eh?

Thinking of making an Oathbreaker and then picking Magic Initiate: Warlock, then picking eldritch blast and Hex. Possibly making a pseudo Hexblade.

I saw someone become oathbroken for simply attacking innocent NPCs.

It will need further testing to see if simply doing the absolutely evil thing isn't enough for both types.

You can also break your oath by attacking the paladin of tyr and his group near the roadside cliffs.

As an oath of ancients paladin i broke my oath by killing on of the goblins with the goblin priestess in the temple, had to reload cause i didnt want to be oathbreaker and didnt expect that to break my oath.
