Reached some kind of red door but couldn't go past it. You may need to finish the Volcano Manor invasion questline to open it. I turned Reyndell to ash before fighting Vargram, so it appears I'm stuck

To access the secret passage behind the throne at the cathedral of the forsaken, you have to take off all weapons and armour.

DO NOT ENTER ITS CHAMBER (i.e. don't open the door at all) if you don't want to infuriate Melina and make her leave. After you open the door cutscene will play directly without giving you an option and Three Fingers will leave a mark on your body. You can still refrain from getting the Crazied Fire Ending with a certain item later but getting the mark will make Melina leave you permanently. You can force close the game immediately after the cutscene if you make the mistake, after reopening the game you will be at the site of grace outside.

Hyetta clearly stated that she will become your maiden if open the door. It is obvious that you can't have two maidens

Thanks for the info I thought you could make a choice. There should be a special achievement for taking the fire then reverse it and pick the other normal endings because not that many people would have figure out that steps, thus saving Melina, then again she want to be the sacrifice so "saving" her is a bit selfish on the player part.

Where do you meet Hyetta? I entered the crypt without meeting her, so I didn't get that conversation. I refrained from opening the door though

If Hyetta's questline is complete, she will appear near the door and will offer you a hint to be undressed.
