The madness and sleep is resistance or is it build up inflicted on you?

It increases your resistance to sleep and madness, but when you get the new resistance it is already "used up" (it increases your current madness/sleep and the max by the same amount), so you need to wait for it to drain. You can test that this is not a problem by almost maxing out Madness with an incantation, and then equipping the shield. It will not cause you to go mad, as the difference between your current madness and max madness is the same.

Does this shield cause blood loss buildup by just equiping it? Or do you need to strike and enemy with it?

It seems like you only need to have it equipped. I triggered blood loss with a dagger without bloodloss by having this in the left hand.

March 7 anon lied, or it was patched. Tested 12 swings with non-build up weapon on a random soldier, didn't bleed.

This shield most likely references Irina, a blind maiden we meet very close to the shield location. Hyetta appearing after Irina's death would explain the "reborn" part of the item description as well.

Doubt it, the shield was probably made way before Irina's "death". Also, she was killed by a misbegotten, not the Briars of Sin spell; you can tell by the cleaver next to her corpse.

I’m pretty sure this is a reference to the game Blasphemous. It shares a similar ascetic to the Sword of the Guilty, which has a person carved in the hilt, the sword is also covered in similar thorns to this shield.
