You get this after turning in two, technically. The first one is just to accept the quest, where you'll gain the talisman.
As a Faith build, this is probably your first incantation that can hit multiple enemies, shooting a wide shotgun blast of rock shards ahead of you. Doesn't deal extreme damage, but at only 10 Faith and costing 7 FP, what did you expect?

damn, this incantation is pretty sick, I've pretty much started using it as my main attack. Practically a shotgun that uses like no FP, fast cast and still deals pretty good damage especially if you are using the clawmark seal
