Of course it demolished him easily, you procced four status effects. I personally beat him with just the Dark Moon Greatsword and a Mimic Tear.

Roll towards him and to the left after a short pause. If you are at distance or rolling away, the ground spikes have two hits. First a small, close hit, then a larger, farther hit. You have to roll AFTER the first one to dodge the second, if you are far away.

Easiest way is to roll in and left, imo.

Jumping will avoid all those ground AoE attacks.

The one where he stomps into the axe? You just run to either side and then dodge at the last second. If you mean second phase, you legit just dodge away from his first leap, then dodge the second slam, then just sprint straight back. It doesn't cover the entire arena, so you can get out of range.
