The "Danger" symbol for his scream shows up way to late. You need to start running the moment he starts lifting his head.

man it's really fun when they reuse the same boss multiple times anyone else remember how much praise the asylum demon, stray demon, and demon firesage in Dark Souls 1 got for their originality

Second phase, keep your eyes on the Headless Ape. Bait his lunge attacks with the big wind up time, deflect, charge attack, Wife lunges too. Dodging the Wife's lunges is harder than deflecting them, so either deflect and run away or deflect and counter. Once the Wife has posture filled up to about half, use shuriken or gouging top to fill the rest. Once you kill the Wife, the rest of the fight is easy, requiring only deflections and careful attacks. Basically deflect everything and only attack when you make Headless stumble forward.
