If you take a look at your Antique Map, accessed by pressing triangle (I think) in the fast travel menu, you'll see the Fountainhead waters pool where you fought him. When the water from the Divine Dragon becomes stagnant, it becomes terribly corrupted, similar to the blood of those afflicted by Dragon Rot. One effect of this are the centipedes, and with them comes undeath. Another effect is poisoning of the environment. This is why the miners in the Sunken Valley carry antidotes, why the area at the bottom of it is a poison swamp, and why the Okami warrior women, who hail from the Fountainhead Palace, are especially weak to the poison status abnormality (for your own good make use of this piece of information in gameplay). The Guardian Ape drank from the corrupted pool of water, so over time the centipede likely grew inside him. I hope that now you can not only see why there is a centipede in the Guardian Ape's bum, but also why the poop from said bum is poisonous. Indeed, Sekiro's game world is quite elaborate, much more so than people tend to give it credit for.
