Speculation here, but what if this is the doll's true form? Gehrman doesnt outfit the doll with Marias Hunting grab, or a replica, because the only thing he made was the face that resembles Maria. What if a brain trust can burst out of the Doll if she decides she wants to fight? The moon presence just left a headless empty doll for Gehrman to improve and it gave it life in the hunters dream once he was done. Problem with this theory is the information on the Dolls clothes, they were heavily detailed and seem to have been made by Gehrman. Or what if in a vision or dream, the moon presence told Gehrman about this style, an outfit these nightmarish denizens already wore. Gehrman replicates it, paying attention to every fine detail, then attaches a face of Maria, a head, and the moon presence customizes this design and orders it to be friendly. Grab-spammers with a crutch mechanic. I ended up just spamming the augur of ebrietas until they died, using blood vials when I needed to. It wasn't efficient, but it got the job done.

The areas where the Lanterns are are the equal of the two Knights with greatbows outside the cathedral in Anor Londo for sheer cheapness. Worst enemy ever.
