Someone vandalized this page, so I decided to revive it and give it a good cleanup.
One thing however that I have yet to confirm is whether or not the vial you get from the Celestial Mob version of Iosefka actually has a different description. A post below claims it did, but from I remember it doesn't change from the first ones you got before. I could be wrong, or perhaps a previous patch secretly changed it, but I'll make revisions if I ended up being wrong.

Went back to her after Father G but before BSB and she will no longer offer this to me, so now I can't get one. Current theory is that once you trigger the old lady NPC who first asks you about a safe place, Iosefka's dialogue changes and she no longer offer the vial.

I don't think that's right, i found the hag before fighting the cleric beast and used quite a few of iosefka:s vials before I beat it. Ir:s probably beating Gascoigne.

In the English language PAL version of the game, this item has the following description: "Blood vial acquired from the lady doctor at Iosefka's clinic. This refined blood, highly invigorating, restores a larger amount of HP. This rare blood vial is a product of a slow and careful refinement process, likely devised by the doctor herself." Calling her "the lady doctor at Iosefka's clinic" would suggest that she is not the titular Iosefka. This is however strange and confusing since, prior to getting this item, she says herself that she is Iosefka.

Based on this...the doctor could had been the fake iosefka...the color is like Kin blood not normal blood. The doctor usurps the clinic as she fears she will be a beast and hopes to ascend. This fake could be tje sister of iosefka. Peering through the door reveals the real one having the same face as the fake. They could be twins. A nice choir hunter andna bad one. The blood offered by the good one is discolored like kin blood after all. She could had experinented on heavily injured victims at deaths door, ppl who wud visit the clinic but cant mske it. Its a war against beast after all. The fake, wanting lots of data fast, needs more victims. Starting w iosefka, she asks the hunter for more, fresh victims.

Considering how much emphasis FROM put in dropping hints to the greater plot in this game, does anyone have a theory as to why one of Iosefka's Blood Vials shows up in a dedicated chest right before the Wet Nurse fight? There's got to be a reason for that. Giving a one-off strong heal from a gameplay standpoint seems rather pointless, it has to be a lore reason (probably tied to the Impostor somehow).

You can get the cord right after rom. My first guess would be that this is a hint that the blood is from a great one. It's also the same color as the slug scholars', brain suckers' and Willem's.

If the wet nurse would feed the child, it would be blood. She has sword hands and the dark rituals at the unseen village, gateway to access the nightmare of mensis is a mess of corpses, weird experiments and death. Maybe all that summoned and sustains Mergo and gives it strength to hold the nightmare? Mergo wud have to age a bit more for human flesh.

Came to this page because I was wondering about this. I always thought Impostor Iosefka was a member of the Church, though. Wouldn't the connection imply that the Impostor was a member of the School of Mensis, instead, since the blood she provided was in their dream? Or was the TRUE Iosefka a member of the School? (I never did get blood from her before coming in the back way to murder her.) This sparked a strange thought. I had just considered the Wet Nurse as a guardian/caretaker for Yarnham's unearthly child. What if it really does FEED the child of itself? I feel like I should have realized this disturbing implication earlier.

"My first guess would be that this is a hint that the blood is from a great one. It's also the same color as the slug scholars', brain suckers' and Willem's." The great ones all bleed red. Orphan of Kos, Mergos wet nurse, Amygdala, Moon presence, Rom, all bleed red. Only the kin bleed serum (white/red mix)

It's not really the color of kin blood which is greyish. Rather it's the color of separated blood plasma (serum) which makes sense given the description of it being highly refined.

The voice from the real Iosefka and the Cainhurst imposter later differs also.
