not sure if the trigger is the number of bosses, having spoken to her just before and just after having killed the shadow of yharnam, that's when the dialogue change triggered for me. (Cleric Beast, Gascoigne, BSB, Amelia, Amygdala, Witch of Hemwick I have also killed so it might be number of bosses also)

Same for me. I also beat those two hunters to the right of the cathedral, and the one with Eileen in the same area Gascoigne's fight was, but those are the only 3 bosses I've killed and she's turned nice. Maybe it happens when you use the tonsil stone to get Amygdala to transport you to the lecture hall? That's when I noticed it anyway. Other things of note I did around that time were bring in the ***** and the skeptic to the chapel, and go through part of the forbidden woods up until you meet the legion guy.

The sky has turned dark, I have beaten the shadows of yharnim bossfight and lit the lantern by byrgenwoth and immediately teleported back to the hunter's dream, she gives me sedatives even though I haven't even enterd the academy.

The first time I noticed her being friendly was after I killed Logarius and talked to the queen and I've pretty much followed this wiki's progress route so I think Logarius would have been the 7th boss I killed.

I may have narrowed it down a bit. For me she became 'motherly' after unlocking the lamp for the Forbidden Woods, and unlocking the shortcut from the area with a bunch of snakes. Not sure if the shortcut is necessary, as I didn't talk to her between unlocking the lamp, and unlocking the shortcut

I used the wall jump early in the game and noticed that the man that gives you the Tonsil Stone and the Lonely Old Woman switch locations. She now is afraid of me and constantly tells me to leave her alone. Wondering if I can still send her to the chapel?

That strange creature sitting on the floor, or whatever it was, had me fooled. I immediately assumed it was a creature and struck it, killing it instantly. It wasn't until after when he said, "I just wanted to help..." that I realized I had possibly made an error. After having watched this, I realize there are all these twists and turns to the story. Honestly, it is aggravating because none of it is direct. When I arrived at the Suspicious Beggar, I did not even have the option of telling him to go anywhere, which is likely a result of these multiple NPC mistakes. Incredibly lame.

Game is not lame, its just people are too dumb to think before strike. You got cut-scene upon entering Chapel and notice there someone sitting next to you, its obvious non hostile. How could you attack him when he did nothing.harm.

You can't lock onto him, and I charged a Kirkhammer sword thrust and hit him, he took no damage and simply got startled, so I have no idea how you managed to kill him without knowing.

When she says "it's your father's blood" she likely says to insult the player, sort of like saying "I killed your father and you drank his blood"
