also,you can get one form the dread behemoth/tyraneant time quest,drop rate is low,not sure exactly the drop rate,but its low

You can also farm Lord Vexxos from Y'jhimel by trading with her after completing "adventurer from another world" quest level 46, the tradable items and farm is all included and self explained in her quest
